Consent and Disclosure Information

Effective Date: April 1, 2025

At Texas Healthy Aging & Hormone Therapy, your safety, autonomy, and trust are our top priorities. This page outlines how we obtain informed consent for all medical and aesthetic services in compliance with the Texas Medical Disclosure Panel (TMDP), HIPAA, and other relevant Texas regulations.

1. Informed Consent Process
Before receiving any medical or aesthetic service, every patient undergoes a thorough informed consent process that includes:

A Good Faith Exam (GFE) conducted by a qualified medical provider

A discussion of diagnosis, proposed treatments, alternatives, and potential risks or complications

A chance for the patient to ask questions and receive clear, honest answers

A signed consent form before proceeding with any treatment

Informed consent is required prior to initiating any procedure, whether it is a hormone injection, lab test, IV therapy, or aesthetic treatment like microneedling or injectables.

2. TMDP List A and List B Disclosures
As required by the Texas Medical Disclosure Panel under 25 TAC §601.2, we provide additional written disclosures for procedures that fall under:

List A: Procedures that require mandatory disclosure of risks (e.g., Bioidentical Hormone Replacement Therapy (BHRT), PDO Threading, or infusion therapy)

List B: Procedures where disclosure is discretionary, but we still provide written information as part of our ethical commitment

These disclosures are reviewed and signed in person as part of the informed consent process. They are not provided online, as every individual’s risk profile must be evaluated directly by our medical team.

3. Aesthetic Medical Procedures
All medical aesthetic treatments (e.g., Botox®, laser therapy, Pixel 8 RF, Spectrum, or Lumines) are considered nonsurgical medical cosmetic procedures and are governed by the Texas Medical Board (TMB) under 22 TAC §193.17. As such:

Patients receive a Good Faith Exam to assess medical appropriateness

Written consent is obtained for every procedure

Risks, expected outcomes, and alternative options are explained clearly

We do not perform any aesthetic procedure without signed authorization and oversight by a licensed medical provider.

4. HIPAA-Compliant Data Sharing
As part of your care, we may share necessary information with:

Diagnostic labs, imaging centers, or specialty providers

Insurance companies or billing services

In-house and authorized third-party care teams

This sharing is done only as allowed under HIPAA and the Texas Medical Records Privacy Act. All parties we work with are required to maintain strict confidentiality and data security.

Your consent forms explicitly state how and when your data may be used or shared for care, billing, or compliance purposes.

5. Parental and Guardian Consent
In accordance with Texas Family Code Chapter 32, minors receiving treatment at our facility (including family medicine, hormone therapy, or certain aesthetic services) must have a parent or legal guardian:

Present at the appointment (unless otherwise permitted by law)

Review and sign all consent forms

Be informed of any treatments, risks, and follow-up requirements

Exceptions apply only in circumstances defined by state law (e.g., emancipated minors, pregnancy-related care, etc.).

6. Right to Refuse or Withdraw Consent
You have the right to:

Refuse treatment at any time

Withdraw previously given consent before a procedure is performed

Request additional clarification about risks or benefits

Review your signed consent forms upon request
